Patient Access (also known as 'online access') allows you to order medication, look at your test results (e.g. blood tests) and access a summary of your medical records.

Step 1 - Fill out the form below.
Step 2 - A member of staff will contact you, asking for you to send in a photograph of some ID e.g. passport or driving licence.
Step 3 - We will activate your online access. Then you simply create an account on Patient Access.

Responsibilities

5. If I see information in my record that is not about me or is inaccurate, I will contact the practice as soon as possible.
