This online form allows you to securely send in your blood pressure readings to the practice.

You should only use this form if asked by a clinician at the practice.

The clinician will let you know if they want 1 weeks of readings or less.

Once you have all the readings, just enter them in the form below and send to us. Simple.

How exactly do I record the blood pressure?

Make sure you have read the short leaflet from the British Hypertension Society [EXTERNAL LINK] on how to measure the perfect blood pressure.

So for day 1 you would measure your blood pressure in the morning and write it in the ‘morning 1st reading’ box.

Then wait 2 minutes and measure the blood pressure again, and write it in the ‘morning 2nd reading’ box.

Do the same in the afternoon.

That’s 4 readings in the day.

Just do the same for the rest of the week until you have 7 days of results, or how ever many you were asked to complete. Then enter them in the form below.
