This online form allows you to securely send in your blood pressure readings to the practice.
You should only use this form if asked by a clinician at the practice.
The clinician will let you know if they want 1 weeks of readings or less.
Once you have all the readings, just enter them in the form below and send to us. Simple.
How exactly do I record the blood pressure?
Make sure you have read the short leaflet from the British Hypertension Society on how to measure the perfect blood pressure.
So for day 1. You would measure your blood pressure in the morning and write it in the ‘morning 1st reading’ box.
Then wait 2 minutes and measure the blood pressure again, and write it in the ‘morning 2nd reading’ box.
Do the same in the afternoon.
That’s 4 readings in the day.
Just do the same for the rest of the week until you have 7 days of results, or how ever many you were asked to complete. Then enter them in the form below.
